She blurted out how she really felt about him and begged him to give their relationship another chance. If he did, then she would seek treatment and do whatever she could to extend her life.

It was tempting for Wyatt, but he ended up pushing her away, claiming that he was committed to Flo.

His guilt will also revolve around what he should do about Sally’s dying request. Does he give her what she wants due to her fake death sentence? Or does he stay committed to Flo?
