She suspected that Katie Logan Spencer (Heather Tom) had told him but Katie managed to convince her that she hadn’t, even though she strongly encouraged her to tell him on her own.

After all, Sally wouldn’t want Wyatt to “find out” the hard way by waking up some morning in bed with her dead beside him! She may be stubborn enough to not tell him until what she thinks is the last minute, but when she really thinks about what it will do to him if he were to find her dead and it was so unexpected, she might realize that telling him would be the best thing to do.

She told Katie that it was getting harder and harder for her to conceal her symptoms, which she knew she had to do when she is at work, but how is she going to conceal them to someone she is living with and sharing a bed with? Ultimately, Sally’s love for Wyatt will likely override her fear of his possible pity for her and she will give him a heads-up that their time together may not be for very long. He will likely encourage her to get second opinions and to fight for her life as Katie did, but the difference is, he has given her a reason to live and a reason to fight – something that Katie was not able to do.

Wyatt will find himself in a sticky wicket should Sally have been incorrectly diagnosed and the disease is not terminal, but for now, just making Sally happy and hopeful is his only goal!
